Eligibility Criteria and Documentation

In all cases, the application must provide proof of hardship along with the demostrated need based upon income level and finances. All hardship events and documentation (lay-off, medical bills, family crisis, etc.) must have occurred within the last 6 months.

  1. Military Hardship- Military deployment of one or both head-of-household, deployment/activation papers and documents will need to be provided to prove deployment. Military personnel returning from active duty with a disability. Proof of circumstances will need to be provided (e.g.: medical discharge form.)
  2. Senior In-Home Care Hardship- Seniors requiring "in-home care" services and other services resulting in difficulty making "out-of-pocket" costs therefore limiting their finances. Proof of these expenses will need to be provided or proof of Community Care Program (CCP) participation.
  3. Foster grandparents raising their minor grandchildren- If a grandparent is the legal custodial guardian of their grandchildren. Applicants must provide the custodial court order or a Medicaid/Kidcare card for grandchild, displaying's name and address as proof of eligility.
  4. Disability Hardship- Household containing a person or persons with a disability, including mental health, that severly limit the household's finances. Applicants must provide Medicaid or Supplemetal Security Income (SSI)documents as proof of eligilibity. Only clients approved for disability benefits qualify. Clients whose disability status is pending, denied, or being appealed are not eligible under this clause, however, if they have documented medical expenses and conditons, they may qualify under the Medical Hardship guideline.
  5. Loss of Employment or Unexpected Loss of Income- Household income impacted by a loss of employment due to the lay-off or termination of employment within the last 6 months. Termination of employment cannot be the result of aplicant's negligence, e.g. being "fired" from a job. However, if they have been approved to received Unemployment Benefits. Termination papers or layoff notice or proof of current enrollment for Unemployment Benefits (check stubs or "UI Claimant Wage Information Sheet") must be provide as proof. FINAL CHECK STUBS ALONE ARE NOT SUFFICIENT!. Unexpected loss of income or reduction in income due to SSI reassessment, tax increase or spousal death. Proof of income loss must be provide. Educational Expenses- Students contributing to the financial income of a family is now attending college/university resulting in a reduction of the household income. Proof of full-time student status (registration documents) AND loss of income (check stubs) must be provided.
  6. Family crisis- Within the last 6 months, household experiencing the death of an immediate family member or terminal illness of an immediate family member. Death certificate or medical papers must be provided for spouse or affected family member.
  7. Family Damage- Unexpected reduction of a household's income due to property damage associated with natural disasters not covered by insurance. Natural disater include flood, tornado, or wind damage to home or vehicle. Submit damage report or insurance claim.
  8. Victimized By Criminal Activity- Unexpected reduction of a household's income as a result of being victimized by criminal activity within the last 6 months. Must provide police report (must have filed within 72 hrs of event) and proof of income loss, e.g. lost work days.

Necesary Documents:

Idetification- The address in the Id it was to be the same address from the account bill. If the address is not the same, show other account, it could be ID, o public documents like medicare with the same address from your utilities.

Income- Gross income from all the member of the household from 18 year and up from the last 30 days, if they are working.

Utility account- The date from the bill has to be within the 30 days. PAY UP TOO $1000, DO NOT EXCEED THE AMOUNT OF THE DUE DATE.

SSN- It is only necessary to determine the eligibility from the income (only if applies to you). Please provide all documents from all circustances. All documents provide dificulties should be within 6 months.
